Academic consultant registration form

Dear Students, 

The registration period for courses will soon be open, please see below a form for consultant in case you wish to address the academic consultant. 

Here are the instruction for registration:

MSc Instruction for registration

PhD Instruction for registration

This form should be sent only in cases where it was not possible to registrant thorough the In-bar system or approval of the consultant is required.

Please first make sure you have filled the student declaration in the In-bar system.

You can send the form via e-mail- in that case, signature is not required.
